Starting July 1st, 2020, we accept patients only with Premera, Blue Cross Blue Shield (limited medical plans) and most of auto state and private insurers (PIP only).
Please be aware that it is your responsibility to know and understand your health insurance policy and its benefits. Your health insurance policy is an arrangement between your Health Plan and yourself. To assist you, we are happy to check your massage therapy benefits. Any quote of benefits for massage therapy we receive by your carrier is not a guarantee of payment. Payment is based on terms, conditions, eligibility and medical necessity as determined by your insurance company at any time of service.
Please notify us if you are pregnant or require additional assistance so we can be sure accommodations are available for your appointment.
We also require all new clients to fill out New Patient paperwork. You can complete the forms in our office or in advance by downloading them below. The forms can be filled online and emailed to firstname.lastname@example.org or by ink and turned in our office when you come in for your appointment.
